After making an appointment with the Institute, you will receive an information packet in the mail prior to your visit. Please review the materials in the packet and complete the enclosed forms.

Things to bring to your appointment

• Bring the completed forms with you to your visit, along with a valid insurance card and photo id.
• Bring a list of your current medications, if appropriate.
• Bring test results pertaining to the affected area, including all old films, MRI’s/CT’s and EMG results.
