There are a few steps necessary to determine if weight loss surgery is the right option for you. The two most important steps are calculating your Body Mass Index (BMI) and determining if your insurance company will cover much of the expense. To get started, follow our easy step-by-step instructions to determine if you are a candidate for weight loss surgery.

1. Education


Body Mass Index Calculator

Body Mass Index (BMI) is a number calculated from a person's weight and height. BMI provides a reliable indicator of body fatness for most people and is used to screen for weight categories that may lead to health problems. This will be part of your evaluation should you elect to have the surgery.

Seminars

If you are interested in minimally invasive weight loss surgery, you will be required to watch our online seminar and complete the quiz (the office will provide seminar for patients with no internet access) Participation must be done prior to a consultation.

2. Qualification

Call Your Insurance Company

Before you begin filling out the paper work, be sure to call your insurance company to ensure you are eligible. This step MUST BE DONE before a consultation is scheduled! Also, there is a non-refundable program fee of $200. Please read below for more information regarding the program fee.

Program Fee

The staff at Covenant Clinic’s Midwest Institute of Advanced Laparoscopic Surgery™ is dedicated to your success. The program fee covers the education and support you are able to take advantage of after surgery through support groups, online communities, and patient support technologies. Education and support are vital components in reaching your goals. The non-refundable program fee of $200 can be paid with cash, cashier’s check or credit card. The first half of the payment is due at the time of your consultation appointment and the remaining $100 is due at your final pre-operative appointment. This fee is not covered by insurance and is non-refundable.
 


3. Consultation

We will need you to complete the online seminar quiz  and the patient information packet before we schedule your consultation. Please submit the patient information packet via email to kim.franzen@wfhc.orgThese things MUST be done in order to schedule your one-on-one consultation!

Insurance Information Handbook

You will receive an insurance information handbook at your consultation. The office will help you submit your information to the insurance company for review and will provide follow-up support as needed. To get the insurance approval process started before your consultation - complete and email us the patient information packet provided below.

We will review your forms to ensure they include all the necessary information we need. If you have any questions or concerns while you are filling out the forms, call us at 319.268.3990. Once the packet process is complete, the approval process for insurance can begin. Our insurance specialist will contact you once all of the information is received.
