Discharge Planning for Inpatients

How long you will be in the hospital depends on many different factors. These include your age, the type of surgery performed, your overall health, and your progression of recovery, etc. Patients requiring further hospital-type care may be transferred from the acute unit to Covenant or Sartori's Skilled Nursing Unit or a skilled nursing facility closer to home for additional care. Those that could benefit from, and qualify for, an aggressive rehabilitation program may be transferred to Covenant Medical Center's Inpatient Rehabilitation Unit.

Whether you are discharged from the acute nursing unit, or one of the units mentioned above, you will be given written discharge instructions at the time of discharge.


Discharge Planning for Outpatients


Your discharge time will be determined by your condition. At that time, a nurse will give you written discharge instructions.

  • You may not be able to visit with your doctor the day of surgery. If you need any information or written work releases from your doctor, your family will need to obtain these for you when they visit with your doctor after surgery.
  • You will need someone to drive you home. Please have transportation arranged ahead of time.
  • Arrange to have a responsible adult stay with you at your home overnight if you have a general or spinal anesthetic.
  • If you have any problems at home, contact your doctor. If you cannot contact your doctor, call Patient Engagement, Wheaton Franciscan's 24-hour health care information department at 319-272-8292.
